Registration Info

St. Mary School Registration Pre-k to 8

Book and Technology fees of $165 per student in grades K-8 are payable at registration. Pre-school students have a $25 non-refundable registration fee payable at registration. To speed up the registration process, you can download print and complete forms found at the bottom of this page. Then bring them to school with you when you come to register your child. It is important to fill out information on the Family Information Form because the lists from the previous year are discarded. All volunteers/parents who want to volunteer in the school must complete the form for a background check, the background check is good for five years. Handbooks will be passed out at registration and required school supplies will be sold.

School Start

The first day of school for Grades K-8 will be August 19, 2011, with dismissal at 10:00 AM. Pre-School will begin on TBA.

Health Requirements

Students in Pre-School, Kindergarten and Grade 6 must have a “Certificate of Child Health Examination” form completed within one year prior to the first day of school. Proof of such examination and immunizations must be presented to the school by October 15th of the current school year. Students entering Kindergarten, Grade 2, and Grade 6 must have a dental form completed within 18 months of May 15th of the current school year. Students entering Kindergarten or entering an Illinois School for the first time in grades 1-12 must have an eye examination within 12 months prior to October 15th of the current school year. These forms are available at school or at your physician's and dentist's office. If you have any questions concerning immunizations, please check with your physician or the Brown County Health Department (773-2714). Brown County Public Health requires a $10.00 fee.

Protecting God’s Children

The Diocese of Springfield requires background checks for all employees and volunteers who have regular or significant contact with minors in any of our school and parish programs. In addition to the background checks, all church personnel and volunteers who spend time with children (even one time) are required to attend a Protecting God's Children Adult Training. You only have to attend this training one time. We will offer a training periodically throughout the year, check www.smseagle.com homepage for upcoming training classes. Please keep in mind, if you want to go on class field trips or volunteer in the school in any way, you are required to attend a Protecting God's Children session.
